Our philosophy is to make referring to us as simple as possible for both you and your patients. You can either download a referral form from our website or email or phone us to request a referral pad. You can fax or email your referral and we will be happy to contact your patient to arrange an appointment. Please also be aware that we do not have x-ray facilities in our rooms so ask that you arrange a referral for any necessary x-rays to be performed so that they are available for us at the time of their consultation.
